Cerebral herniationのリスクがあってもlumbar punctureをしなければ治療方針が立てられない重症CNS感染で，LPなしですべての病原体をカバーする治療をしては薬剤ストックがなくなるような環境では，LPによる突然死のリスクを本人家族に背負ってもらってでもLPしなければならない．
